Accountable Care Organization (ACO) Prompted by concerns that the U.S. healthcare system suffers from serious gaps in quality and widespread waste, many reform efforts have focused on holding individual providers accountable through performance-measurement and pay-for-performance programs. By focusing on individual clinicians, however, such efforts may overlook quality problems attributed to poorly coordinated care.

CMS Issues 2015 Performance Data for Medicare ACOs
Medicare Accountable Care Organizations qualified for more than $466 million in shared savings in 2015 by meeting quality standards and their savings threshold, the Centers for Medicare & Medicaid Services announced last week. Eight Pioneer ACOs generated more than $37 million in shared savings and four generated losses. In addition, 119 Medicare Shared Savings Program ACOs earned $429 million in shared savings. Pioneer ACOs are early adopters of coordinated care and assume greater performance-based financial risk. ACOs with three years of experience in the program were more likely to earn savings (42%) than those participating one (22%) or two years (37%). Pioneer ACOs increased their mean quality score to more than 92%, an increase of more than 21 percentage points since the first year. MSSP ACOs improved on 84% of the quality measures that were reported in both 2014 and 2015.

FDA Issues Safety Recommendations for Programmable Syringe Pumps
On August 25, the Food and Drug Administration issued recommendations to reduce safety risks when using programmable syringe pumps to infuse therapies at low rates, when a lack of flow continuity can result in serious clinical consequences, including delay of therapy, over-infusion or under-infusion. Since March 2013, the agency has received more than 300 Medical Device Reports associated with programmable syringe pump use. Of the 100 MDRs that provided information on the infusion rates, the majority noted infusions at rates of 5 mL per hour or less.

CDC Issues Summary of Key Zika Considerations for Healthcare Settings
The Centers for Disease Control and Prevention has released a summary of key considerations for healthcare facilities, including hospitals and health systems, as they prepare to receive patients potentially infected with the Zika virus. The document outlines nine steps healthcare facilities should take and also notes steps healthcare settings can take to educate patients and their families about the disease, transmission and steps to avoid infection. CDC also offers a 24/7 Zika hotline, at 770-488-7100, that can provide rapid access to CDC clinical experts, as well a dedicated e-mail address for pregnancy-related Zika questions at zikamch@cdc.gov. ONC Issues Draft 2017 Interoperability Standards Advisory
The Department of Health and Human Services’ Office of the National Coordinator for Health Information Technology last Monday released for public comments its 2017 Draft Interoperability Standards Advisory, an updated list and assessment of the standards and implementation specifications available to meet clinical health IT interoperability needs. The advisory is a non-binding document, but the standards and implementation specifications may be considered for rulemaking or other federal requirements. ONC particularly seeks comments on the references to more than one standard or implementation specification for a specific need. Comments are due Oct. 24. For more information, see the ONC blog post.

AHRMM Report: CQO and the Triple Aim: Supply Chain’s Strategic Connection 
Earlier this month, AHRMM released the latest Cost, Quality, and Outcomes (CQO) Report: CQO and the Triple Aim: Supply Chain’s Strategic Connection. This report contains seven case studies on healthcare organizations that have successfully aligned their supply chains to the CQO Movement and the Triple Aim through initiatives such as supply and process standardization to reduce surgical site infections (SSIs), to logistical improvements aimed at delivering better patient care to the bedside and beyond.
